We currently have an opportunity for a:
Clinical Case Manager
  Regular, Full-Time – 35 hours*/week  - Simcoe Program
*The successful candidate will be required to work some evenings a week based on client needs.
Hourly Rate: $34.84-36.59-38.41
 
Job Summary:  
The Clinical Case Manager position is an integral part of a busy, dynamic and growing clinical program at Kinark Child and Family Services, Community Based Child and Youth Mental Health program in the Simcoe region.  The Clinical Case Manager will be expected to exercise strong familiarity and knowledge of case management principles and practices and have awareness of community resources and services available within the children’s mental health sector. 
Reporting to the Supervisor of Client Services, the Clinical Case Manager will be responsible for completing risk assessments, planning with clients and supporting the interdisciplinary clinical team in the co-ordination and provision of client intake, service delivery, discharge, and postvention, as required.  
Duties & Responsibilities (include but are not limited to):
·        Connect and maintain collaborative relationships with community partners by liaising with child welfare, education, health and other community stakeholders and partners
·        Manage pod wait list and support clients while waiting for services which includes conducting risk assessments and necessary safety plans and support clients on wait list.
·        Contribute to assignment of cases to pod therapists, expediting high risk clients into service
·        Participate in program area admission to service process in collaboration with assigned therapist, completing such tasks as preparing consents and facilitating the sharing of relevant collateral information.
·        Identify and facilitate referrals to internal resources (i.e., psychiatry, psychology, therapeutic groups, etc.) as well as to external resources (i.e., services provided by community partners)
·        Coordinate and actively participate in client case conferences and treatment meetings
·        Collaborate with other members of the treatment team and participate in interagency/team meetings through attendance at all clinical and interdisciplinary team meetings
Qualifications:
·  Master's degree in Social Work is required.
·        Current member and in good standing with the Ontario College of Social Workers & Social Service Workers, is required 
·        Minimum 3 years’ experience working in a children’s mental health setting
·        Current knowledge of the Simcoe Region community resources is an asset.
·        Experience with providing a broad range of clinical services and demonstrated effective work with a child and adolescent population who present a variety of clinically challenging issues
·        Excellent clinical assessment, formulation and treatment planning skills is essential
·        Strong case management and crisis intervention skills.
·        An understanding of the complexities of working with multi-stressed individuals and families is required.  Experience and knowledge in crisis intervention is an asset. 
·        Strong organizational and time management skills,
·        Strong Relationship Building, Engagement and Meeting Facilitation Skills.
·        Computer competency and experience working with diverse cultures are considered as assets for this position.
·        Travel is required for this position, the successful candidate must have a valid  Ontario driver’s license, access to a reliable vehicle and be willing to obtain automobile insurance which includes driving for business purposes with a minimum liability of one million dollars
·        Satisfactory Criminal Record Check (including a Vulnerable Sector Screening) and Pre-Employment Medical. 
·        Candidates Bilingual in English and French will be given special consideration.
